Every patient should bring their photo ID and most up-to-date insurance card(s) to each appointment. Along with their photo ID and insurance card(s), new patients should also bring the following:

  • Social Security card;
  • Co-payment amount;
    Birth certificate and shot records for children;
  • Copy of medical records, if possible;
  • Prescription medicine bottles;
  • 3 recent pay stubs OR a notarized letter from an employer for household income (for patients who qualify for our sliding fee discount); AND
  • Appropriate sliding fee payment (for patients who qualify for our sliding fee discount).

Services are provided on a sliding fee scale based on the patient’s income and family size. The amount you pay if you qualify for the sliding fee scale will be determined once new patients bring 3 recent pay stubs OR a notarized letter from an employer for household income to their appointment.

Yes! We encourage all members of the community, not just our patients, to compare prescription prices at our pharmacies and use our services.

Yes! We have certified representatives who can assist you for free. You can also visit https://www.healthcare.gov/screener/ to learn if you qualify for special enrollment.
